Tax Preparation Checklist
Below you will find information you will need in order to file your taxes.
Personal Information

Your Driver's License

Your Social Security card or tax ID number (if you have a business)

Your spouse’s Driver's License and Social Security card or tax ID number (if you are married and they have a business)

Dependents Social Security cards and date of birth​
Forms You Need
If you receive any of the forms below, you will need to bring them in order to file your taxes:

W-2 from your employer

W2- G for gambling income

1099 MISC or 1099 NEC (if you have a business or self employed)

1099-R if you have retirement income
-
1099 - SSA if you receive Social Security Benefits

1099 - INT for interest income

1099 - DIV for dividend income

1099 - B or 1099 - S for sales of stock or property

1099 - G for unemployment income

1098 - E for student loan paid interest

1098 - T if you (or your dependent) attends college
-
1095 - A for Marketplace Health Insurance coverage

Form 5498-SA showing HSA contributions
Self Employed Information
If you own a business or are self employed please bring the following in order to file your taxes:

Forms 1099-MISC, Schedules K-1, income records to verify amounts not reported on 1099s

Records of all expenses; check registers or credit card statements, and receipts

Amount paid for preparation of last year’s tax return

Business related expenses (dues, publications, tools, uniform cost and cleaning, travel)

Business-use asset information (cost, date placed in service, etc.) for depreciation

Office in home information and Forms 1098 or other mortgage interest statements , if applicable

Log showing total miles driven for the year (or beginning/ending odometer readings), total business miles driven for the year (other than commuting), and the business purpose of the mileage

Amount of parking and tolls paid

If you want to claim actual expenses, receipts or totals for gas, oil, car washes, licenses, personal property tax, lease or interest expense
Rental Property Information

Records of income and expenses
